Demi Moore Enters Hospital Due To Substance Abuse

Demi Moore was hospitalized Monday night due to a substance abuse problem. A 911 call was placed and upon response she was take directly to the hospital.

Law enforcement stated that a 911 call was placed Monday night at around 10:45. The call prompted paramedics and they arrived on the scene. The paramedics assessed Moore's condition for about a half hour, then transported her to a local hospital.

Sources say that she was hospitalized in order to "seek further professional assistance" for substance abuse.

Demi's representative told TMZ that she was seeking professional assistance to treat her exhaustion and seek to improve her overall health.
